You can use this form to order crew shoes. We now have two different suppliers for crew shoes – WorkMode and Bata. You can choose any of the “models” on the page below. The company will take the cost out of your pay over 3 weeks. If you need more time to pay, please see your Restaurant Manager.You are required to complete and sign the order form. Click here to download the Order Form. Please print this and complete it and email a copy to the email address on the bottom of the form.
The Bata shoes are quite large sizing so you may find that you need to order a smaller size than you would normally fit. If you don’t know your shoe size, or want to double check, ask your manager to provide you with the shoe size gauge.
Once the form is completed and submitted, your shoes should arrive at the restaurant within 5 working days. If there are any problems, you will be contacted at the email address you used to order (or the email address we have recorded for you on file if there is an issue with the email address you ordered with.)
